Nomination Deadline is January 14, 2012
The Family Medicine Physician of the Year is selected annually from nominations by MAFP members, component chapters, and county medical societies. The award is presented to a physician who exemplifies the tradition of the family doctor and the contribution of the family physician to the continuing health of Michigan citizens.
Criteria
As a guideline, the selection will be made based on the following criteria:
- Nominee is an active member in good standing of the MAFP.
- Nominee spends at least 50% of his/her time in patient care.
- Letters from patients in support of the nomination are encouraged.
- Letters from physician colleagues/hospital administrators, etc., are also encouraged. - Nominee participates in community affairs.
- Letters from community leaders are encouraged.
A current curriculum vitae, business photograph and 3 letters in support of the nominee from patients and colleagues must be presented with the nominations.
All previously nominated physicians are eligible for reconsideration, but must be re-nominated.
The Award will be presented during the 2012 Scientific Assembly at the Somerset Inn, Troy.
