Event: Knowledge Self-Assessment on Health Behavior

Upon completion of Part A, the physician will be able to demonstrate knowledge in obesity, smoking, nutrition, exercise, substance abuse, and sexual health. Demonstrated skills gained will include:

  • Taking a history  for a health behavior case
  • Performing a physical examination for a health behavior case
  • Performing a laboratory evaluation for a health behavior case
  • Managing a health behavior case

Schedule: 8:30 a.m. - 12:30 p.m.

Breakfast will be served on-site at 8:00 a.m. The KSA will begin promptly at 8:30 a.m. and conclude by 12:30 p.m., during which time you will collectively complete the 60-question KSA with your colleagues in a distraction-free, group discussion format facilitated by Dr. David Walsworth. At the session's conclusion, MAFP staff will submit your score to ABFM, and in turn, ABFM will report your CME credits to AAFP.  Please note: The registration fee includes breakfast and refreshments; it does not include ABFM fees or recertification fees.

Mode / Location for Attendance: You have two options to attend this Live CME activity

  1. Attend in-person at MAFP Headquarters, 2164 Commons Parkway, Okemos, MI 48864, or
  2. Attend virtually via Zoom. There is no additional charge for virtual attendance.

Cost: Member Early-Bird Rate $199  (through November 22, 2018)

When registering, MAFP members must first log in to receive the member rate.

CME:  This Live activity, KSA Health Behavior, with a beginning date of 12/08/2018, has been reviewed and is acceptable for up to  8.00 Prescribed credit(s) by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Register: $199 Member through November 22, 2018; $249 Member rate thereafter. Non-Member rates also available.

Cancellation: All cancellations and refund requests must be submitted in writing to the MAFP office. An administrative fee of $50 will be deducted from any and all cancellations and refunds. The final date for refunds is November 22, 2018. No refunds will be granted after this date.
